Questions

Which characters must be escaped?

& and < always, > for safety, and " and ' inside attribute values. "Only &, <, >, quotes" does exactly that; the rest of the text stays readable.

When do I need all non-ASCII as entities?

Rarely — when a page or e-mail template has to stay 7-bit ASCII. Characters with a name become é, —; the rest become numbers like Ж.
